Supporting via telephone and IT portal advanced clinical triage of patients in their normal place of residence, determine if a two-hour urgent response is required to support admission avoidance, support appropriate alternative pathways of care with community health and social care services. Support the access into acute portals where it is appropriate to navigate an alternative to Emergency Departments.
The post holder will provide advanced clinical expertise and leadership to the Integrated Care Coordination for Urgent Care to coordinate and navigate appropriate pathways of care to support patients to receive the right care at the right time in the right place.
The Advanced Clinical Practitioner will work autonomously to facilitate advanced, evidenced based care within community settings as part of an integrated health and social care approach.
The Advanced Clinical Practitioner will work in collaboration with other health and social care professionals and system partners across primary, intermediate and secondary care to support access into appropriate services
To encompass the four pillars of advanced clinical practice; Leadership and Management, Education and Research with the demonstration of core capabilities and area specific competence. The role will also include Clinical Audit, Teaching, Clinical Supervision and Continuous Professional Development and Research.
Main duties of the job

Clinical
Work in accordance with the Nursing and Midwifery Council/Allied Health Professional Code of Professional Conduct. The Post holder will be responsible for providing advanced clinical care expertise in care delivery and clinical decision making to patients within scope of practice through the advanced clinical competency framework.
To promote and lead on physical health monitoring for a defined caseload of patients, requesting all necessary investigations and, where appropriate, accurately interpreting, communication.
Provide clinical leadership and expertise in the triaging and supporting the triage in the ICC for Urgent Care from health and social care professionals with patients in need of support of coordination and navigation of care, General Practitioners, Nursing and Care homes and the management of category 3 and 4 cases via the West Midlands Ambulance Service CAD portal.
Report and raised concerns of quality and safety and Safeguarding. Understanding of trust policies for the safeguarding including awareness of mental capacity, DOLs and the principles of prevent.
Prioritise health problems and intervene appropriately to assist patients in complex, urgent or emergency situations including the initiation of emergency care.
Empower professionals/patients/carers to make informed decisions and delegate care appropriately within the services available.

Job responsibilities
Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a masters level award or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence.
The Advanced Clinical Practitioner will therefore be required to embody the ability to manage clinical care in partnership with individuals, families and carers. This will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance peoples experience and improve outcomes.
Advanced nurse practitioners are highly experienced and educated members of the care team who are able to diagnose and treat conditions and refer onwards to appropriate specialists if needed.
As such the ACP within the ICC for Urgent Care will be expected to:
Hold an appropriate Masters level award with appropriate modules in Advanced Health Assessment and in Non-Medical Prescribing
Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, in line with core professional competencies of the Advanced Clinical Practitioner, including full participation in appraisal and supervision, and acting as role model for self-development
Provide autonomous clinical assessment and treatment, demonstrating clinical expertise at an advanced level.
Contribute to the delivery of programmes of quality improvement.
Provide advanced and expert level of practice (including non-medical prescribing)
Provide clinical leadership and management leading to the provision of safe, quality, effective care
Improve workforce knowledge and practice.
Lead on improvements in the quality of care and safety through service development, evaluation and audit
