We are seeking a dynamic and enthusiastic General Practitioner to contribute to our innovative, multi-disciplinary approach to patient care.
About the Role
As part of the ARRS initiative, you will work closely with a team of healthcare professionals, including Clinical Pharmacists, Paramedics, Physician Associates, and Social Prescribing Link Workers, to enhance the services we provide to our diverse patient population.
Key Responsibilities
* Deliver high-quality, patient-centered care in collaboration with the PCN team.
* Support the management of long-term conditions, medication reviews, and acute presentations.
* Contribute to clinical innovation and service development.
* Play a key role in improving access and outcomes for patients within the network.
* Engage in population health management and proactive patient care planning.
