Job summary Wentworth 1 Primary Care Network is made up of several GP practices in the Rotherham area. We are recruiting a number of Advanced Clinical Practitioners to work in our friendly GP Practices. We are recruiting for an Advanced Clinical Practitioner to work within our PCN. 2 of our Practices both require 18.5 hours per week. We can put these together to create a full time role, or you may wish to apply for just one practice either Shakespeare Road Medical Centre or York Road Surgery. The Advanced Clinical Practitioner will utilise a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions. The post holder will demonstrate critical thinking in the clinical decision-making process. Clinical care will include new patient clerking, initial history taking, face-to-face assessment, examination, diagnosis, treatment, management, referral, and evaluation of care. You must possess a Digital Badge as this role is funded via the ARRS scheme. Main duties of the job Work autonomously and be accountable for his/her own professional actions. Undertake clinical decisions at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care service. Assess undifferentiated patients with a range of acute, non-urgent and chronic medical decisions. Obtain a clinical history, undertake appropriate physical examinations, and investigations, formulate differential diagnosis and carry through management treatment plans ensuring relevant follow up. Undertake pre booked emergency clinics, face to face consultations, telephone or video consultation. Working as part of a larger primary care network to improve patient care and ensure more efficient ways of working. Promote a learning environment for patients, nurses and other health professionals.
