We are seeking newly qualified GPs who have not held substantive posts to work within Hammersmith and Fulham Central PCN on a fixed term contract.
Main duties of the job
This is a position for newly qualified GPs (within 2 years of CCT date & not having held a substantive post before) to work within the Hammersmith and Fulham Central PCN on a fixed term contract until 31.03.25. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
This post will require you to travel between different practices within the PCN as and when required and will require you to be registered with the General Medical Council (GMC) and hold a certificate of completion of training within two years. This role does NOT apply to General Medical Practitioners who are beyond the second anniversary of their certificate of completion of training.

4. Work with member practices, the commissioner & othernetworks to enhance and optimise local improvement programmes aligned to local,regional and national priorities
5. Enhance and optimise local initiatives that enable deliveryof the PCNs agenda, working with commissioners & other networks to reflectlocal needs & to ensure initiatives are coordinated
6. Work closely with other primary care, health & socialcare providers, local commissioners & LMCs
7. Participate in PCN research studies & work with localprimary care research networks & research institutions
8. Contribute to the strategy & wider working of the ICS/ICB
Job responsibilities

Only Continued thereafter subject to confirmationof the continuation of the GP ARRS Scheme from NHSE.
The post-holder will manage a caseload and deal witha wide range of health needs in a primary care setting, ensuring the higheststandards of care for all registered and temporary patients.

PrimaryResponsibilities
Inaccordance with the PCN/Practice timetable, as agreed, the post-holder willmake him/her-self available to undertake a variety of duties including surgeryconsultations, telephone/video consultations and queries, visiting patients athome, checking and signing repeat prescriptions and dealing with queries,paperwork and correspondence in a timely fashion.
Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within the practice orexternal organisations.
Assessingthe health needs of patients with undifferentiated and undiagnosed problems.
Screeningpatients for disease risk factors and early signs of illness.
Contributing to QOF requirements and recording data appropriately.
Contributing to PCN requirements and recording data appropriately.
Inconsultation with patients and in line with current practice disease managementprotocols, developing care plans for health-related illnesses.
Screeningpatients for health education.
Admittingand discharging patients to and from the caseload and referring to other careproviders as appropriate.
Recordingclear and contemporaneous consultation notes to agreed standards.
Collectingdata for audit purposes and contributing to the practice audit programme asrequired.
Compilingand issuing computer-generated acute and repeat prescriptions using ESP(avoiding physical or hand-written prescriptions whenever possible).
Prescribing in accordance with the accepted prescribing formulary (orgenerically) whenever this is clinically appropriate.
Takeresponsibility for some aspects of the clinical work within the team, forexample: coordinating aspects of governance, QOF, enhanced services, CPD andkeeping clinical protocols up to date. In general, the post holder will beexpected to undertake all the normal duties and responsibilities associatedwith a GP working within primary care, as set by Royal College of GeneralPractitioners, General Medical council, all other Royal colleges and councilsapplicable to the staff within the practice, Department of Health, ClinicalCommissioning Groups and all other regulatory and standard settingorganisations.
CollaborativeWorking Relationships:
Foster andmaintain strong links with all services across the PCN and neighbouringnetworks.
Exploresthe potential for collaborative working and takes opportunities to initiate andsustain such relationships.
Demonstrates use of appropriate communication to gain the co-operation ofrelevant stakeholders (including patients, senior and peer colleagues, andother professionals, other NHS/private organisations e.g. ICBs).
Canrecognise personal limitations and refer to more appropriate colleague(s) whennecessary.
Liaiseswith other stakeholders as needed for the collective benefit of patientsincluding but not limited to:
o Patientsand their representatives.
o GP, nursesand other practice staff.
Otherhealthcare professionals including community pharmacists, pharmacy technicians,social prescribers, first contact physiotherapists, physicians associate andparamedics.
oCommunity pharmacists and support staff
oOther members of the medicines management (MM) team including pharmacists,technicians and dieticians
oLocality/GP prescribing lead
oCommunity nurses and other allied health professionals
oHospital staff with responsibilities for prescribing and medicines optimisation

Work withyour line manager to undertake continual personal and professional development,taking an active part in reviewing and developing the role andresponsibilities.
Involvedin one to one meetings with line manager monthly to discuss targets andoutcomes achieved.

Experience
* Experience of working within the NHS, with general public & team working
* Well developed IT skills and an enthusiasm for new applications and software
* Good time management and highly organised

* Able to obtain and analyse complex technical information

* Eligible to work in UK
* Newly qualified GP (no more than 2 years qualified) with no previous substantive post
* Able to carry out duties with or without premises adaptation

* Other postgraduate qualifications (Minor Surgery, Education, Family planning etc)
