The post holder will diagnose, treat and manage patients with a wide range ofdermatological conditions referred to the community Skin Health Service by GPs in East and North Hertfordshire including onward referrals of 2ww to secondary care. The postholder will undertake minor operations and dermatology consultations including cryotherapy and injection clinics within a community setting.

The post holder will work as part of a team of Skin Health Service doctors, with support from the Clinical Lead.
The Skin Health Service comprises of a clinical lead (who is also the clinical supervisor for the GPwER and a senior visiting fellow at the University of Hertfordshire), an associate specialist, 3 GPwERs and GP Fellows. The service is supported by an operational lead, a service coordinator, and a team of administrators. HCT are currently advertising for a consultant dermatologist to join the team, in a joint post withThe East and North Hertfordshire Hospitals NHS Trust.The Skin Health Service provides a single point of contact for all skin health referrals (i.e. dermatology and minor surgery), a robust triage process and the provision of specialist community dermatology and minor surgery, where the patient can be seen by both a dermatology specialist for assessment and have an operative procedure at the same time where appropriate.

• To undertake new and follow up consultations including a full assessment of the medical condition, the patients concerns and expectations.
· To prescribe treatment for the presenting complaint.
· To ensure that patient or carers are informed fully about the condition, answer any questions, supply written information if available and involve them in decisions about the treatment.
· To document notes within the electronic patient record – SystmOne.
· To make onward referral to secondary care where appropriate.
· To create a letter to the patient’s GP at the time of the consultation.
· To discuss or refer a patient for a secondopinion with the clinical lead within the community service, as required, avoiding practice outside his/her level of competence.
· To undertake appropriate surgical procedures
·To order and review appropriate investigations, making sure that histology & pathology requests are correctly completed with full clinical details.
· To undertake the triaging of referrals as requested
· To work with other members of staff to deliver a high quality, patient centered service which will include attendance at clinical /operational, educational and MDT meetings.
·To participate in Skin Health Service clinical audit and evaluation of clinical outcomes.
·Comply with GMC standards of Good Medical Practice and the NHS constitution.
· Participate in the clinical supervision programme within the Skin Health Service via monthly education meetings including working with the clinical lead to identify gaps and CPD requirements.
· To undertake all required GPwER training and development to maintain skills/accreditation.
