Job summary We are pleased to offer an exciting opportunity for a Nurse or AHP to apply for a band 7 Diabetes Team lead within North Cumbria Integrated Care Trust This is a fantastic opportunity for a dedicated and enthusiastic person who is passionate about Diabetes to utilise their clinical and leadership skills while supporting the development of the Service across North Cumbria The post holder will be an integral member of the Diabetes Service specialist team, providing leadership and line management to a specialist team of professionals and also care, treatment and education to diabetes patients and their families. This will involve working closely with the community network of health care professionals to deliver an integrated package of care between hospital and primary care. In conjunction with the Lead clinician and Service Manager the Team Lead is accountable for the quality of diabetes Services provided by the Trust. The post holder will be based in Carlisle however will be required at times to travel across North Cumbria. The post is for 30 hours per week. Main duties of the job Provide clinical input, demonstrating advanced competence in complex decision making, assessment and management of clinical needs of patients with Diabetes and often complex needs. Actively manage and supporting the development of individuals and the team through appraisal, personal development planning, coaching and mentoring Provide professional assurance and quality governance in the service Leading on the implementation of clinical protocols/pathways in identified service lines Further develop rehabilitation and recovery focused services to enable more people to be independent for longer, manage their health needs and lead healthy, active lives in their community with a key focus on Diabetes.
