The role of theIntegrated Care Team Nurseis to conduct the nursing element of the comprehensive geriatric assessmentsand nursing procedures for our patients as well as exercising leadership and management of a defined nursing team within theEast Integrated care teams. Todeliver skilled evidence-based nursing assessment and relay this information to the other members of the MDT ina timelyand concise manner.Toensure safe clinical practices andhigh standardsof care aremaintainedto meet service demand.
This rolewillalsobe responsible forthe smooth running of a PICC clinic and catheter clinics, linking in with the relevant specialist teams.
Service provision isduring working hours between Monday to Friday.37.5 hours per week, working across3sites.Reasonable flexible working requests may be considered.
• Conduct the nursing element of a comprehensive Geriatric Assessment for our patients, to include patients whomayfor examplebe frail, at risk of falling, have history of falls, balance issues, elderly etc.
• Overseeing the Catheter clinic, PICC clinic and Holter devices, including managingthe demand andtheappointment diary.
• Triage patients to ensure referrals meet the service criteriaandidentifywhen the patient needs to be discharged from the service appropriately
• Meet the service demands including reprioritising clinical and non-clinical workload, applying contingencyplansand flexing the service, and where necessary escalating to the Team Lead.
• Provide clinical support,leadershipand guidance to junior staff, including Band 5 nurse, and health care assistants
• Communicate effectively with patients, carers, family members, including those with communication impairments, challengingbehaviouror cognitive deficits, as well as those whose first language is not English.
• Documentation to be completed for all interactionsregardingpatientsin a timely manneronRiO, ensuringdetailed,accurateand confidential patient records according to Trust policies
• To ensure the workload is evenly distributed amongst the nursing team and delegate work asdeemednecessary.
