LDA Hospital at Home is an innovative service being developed under the Thames Valley Provider Collaborative, focusing on young people with moderate to severe learning disabilities and/or autism-related functional impairments. The service aims to support young people at risk of home or placement breakdown, preventing unnecessary admissions and stepping down from inpatient care.
By utilizing new technology and providing care closer to home, the service reduces admissions, length of stay, and waiting times, offering earlier discharges from inpatient care. We aim to minimize the burden on families, enhance engagement with community teams, encourage early therapeutic home leave, and ensure the best care pathway for each young person.
The role is based at the Meadow Unit at our Warneford Site in Oxford. We offer flexibility in hours and welcome applications for both part-time and full-time positions.

Main duties of the job
As the CAMHS community nurse in the team, you will work as part of the multi-disciplinary team to assess, plan, implement and evaluate the specialist health care needs of children and young people who have a learning disability and/or Autism.
The team will have a phased implementation.
During Phase 1; the senior team will be offering consultations to the network and contributing to individual cases. They will be preparing for the training and preparation for the rest of the team.
During Phase 2; you will be part of discussions for planned implementation of agreed interventions, on a small number of agreed pilot cases. You will be part of the team to put the agreed interventions in place, working collaboratively with the young person, family and other relevant professionals in the network for that individual. You will have an identified supervisor and attend both 1-1 and group supervision/debriefs for these complex cases.
During Phase 3: You will continue to be part of the MDT as the team expands, continuing to contribute to the team process and development.

LDA Hospital at Home is a specialist service working with patients up to 18 years old with moderate to severe learning disability and autism. The Thames Valley Provider Collaborative is a network of service providers covering Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and Bath and North East Somerset (BaNES), working collaboratively to provide high quality care for the most unwell young people as close to home as possible.
We are currently recruiting our multi-disciplinary team (MDT) who will work with young people diagnosed with moderate to severe learning disabilities and autism, who are at risk of admission due to mental health difficulties.
You will be based in Oxford in our new offices located above the meadow unit on the Warneford site.

* You will have the opportunity to be part of developing an innovative LDA Hospital at Home team, with the focus of supporting young people to stay at home with clear packages of care around them.
* We are an inclusive and forward-thinking team, there will be a strong focus on training, CPD, supervision and staff wellbeing.
* This role could be full time or part time (minimal 22.5 hours) with some flexibilities.
* Access to transport for home visits and community visits is essential. If necessary adjustments can be considered in line with the Equality Act 2010.
Person specification
Knowledge
* Knowledge of physical and mental health conditions affecting people with learning disabilities and/or Autism, and how these are managed.
* Working knowledge of the Mental Capacity Act.
* Understanding and experience of Risk Management including within the identified client group for the LDA H@H team.
* Awareness of DOLS
* Awareness of LEAPS and CETR’s
Qualifications
* Registered Learning Disabilities Nurse or RN Adult or RN Mental Health or RN Child with evidence of knowledge and experience of supporting the health needs of people with learning disabilities and/or Autism.
* Additional qualifications in evidenced based therapeutic interventions.
* Mentor course completed or willing to complete.
Further training
* Ability to complete comprehensive nursing assessments and devise nursing interventions.
* Ability to communicate effectively with people who have difficulty understanding.
* Ability to work under indirect supervision.
Experience
* Experience of working with people with learning disability and/or Autism from previous employment, training placements or voluntary work.
* Experience of working in an MDT.
* CAMHS experience or working with children and young people in other health settings.
