Within South Sefton PCN our aims are to:
1. Improve resilience in General Practice
2. Build a stronger and more sustainable general practice service across the Primary Care Network footprint
3. Facilitate collaborative working between all Primary Care Network practices
4. Engage with local health and care providers to develop place-based care to assist in the transformation of local services to improve the health and wellbeing of the Primary Care Network population
5. Work with Patient Participation Groups to improved patient access, experience and quality
6. Reach out to strengthen and develop working relationships with non-NHS community groups
7. Develop signposting with Primary Care Network practices to streamline the patient journey to enhance more achievable and sustainable outcomes
8. Further develop digital technology as a primary resource for practices and patients
9. Work in collaboration with the local GP Federation to build and strengthen relationships

This role may vary dependent on the key priorities of the PCN. This may change in line with NHS England and changes in the key priorities within South Sefton PCN.
Nurse Associate Qualification is essential. You will need this otherwise you will not be able to fulfill the role.

Essential criteria
10. Registered Nurse Associate qualification
11. Nursing Associate foundation degree qualification
Desirable criteria
12. Delivery of cervical screening
13. GCSE Grade A to C in English and Math’s or equivalent level
Skills, Knowledge and experience
Essential criteria
14. Experience of working within the NHS/ Primary Care
15. Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
16. Experience working in an NHS setting and/or experience of working with or in general practice
17. Understands and acts in line with NMC professional standards for practice contained within The Code
18. Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
19. Understanding the importance of following procedures and treatment plans
20. Proven ability to recognise and manage risk
21. Ability to take part in reflective practice and clinical supervision activities
22. Ability to organise and prioritise own and delegated workload
23. Excellent interpersonal skills and ability to interact professionally with people from diverse cultural, racial, ethnic, gender, and socioeconomic backgrounds
24. Ability to show empathy and be non judgmental towards individuals
25. Accountability must understand the concepts of accountability and be confident to take guidance from a more senior registered practitioner

29. Proficiency in navigating computer systems with the ability to learn new data systems quickly
30. Recording and understanding clinical observations
31. Long term conditions (LTC) monitoring and Monitoring of LTC/annual reviews

33. Maintaining CPD portfolio and understanding requirements for NMC professional revalidation
34. Chaperoning duties
