Working closely with multi-disciplinary teams across health, social and voluntary sectors you’ll deliver high quality, individualised, evidence-based care to all patients, maintaining confidentiality, privacy and dignity. You’ll care for patients and their families in the way that you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work.
As an Occupational Therapist, you will work within the Intermediate Care Team, collaborating with our team of Physiotherapists, OTs, Nurses, and Rehab Assistants to deliver high quality care to patients in the community by assessing their needs and implementing a treatment plan to encourage their independent living and prevent hospital admissions.

Main Responsibilities: Within this role, you will:
1. Deliver high quality, individualised, evidence-based care to all patients.
2. Teach patients, carers, and peers.
3. Ensure that therapy input is integrated into the patients’ overall care plan.
4. Operate as an autonomous practitioner working without direct supervision and at times lone working.
5. Provide support to unqualified staff and students.
6. Work with managers to develop the service in line with patient need, local and national developments.
7. Maintain accurate records in line with local standards and The Royal College of Occupational Therapists standards.
8. Learn, develop, and adapt to change evidenced in CPD.
9. Operate in a team and contribute to team decisions.
10. Manage own caseload and prioritise work.
The Ideal Candidate: You will have:
1. HCPC (Occupational Therapy) registration.
2. BSc/Diploma in Occupational Therapy or equivalent.
3. The ability to work flexibly across specialties as need arises.
4. The ability to carry out a range of assessments and procedures in order to write and evaluate treatment plans.
5. The ability to identify and set clinical goals with patient and/or carer.
6. Experience of working within a Health Care organisation at pre-registration or post-registration level.
7. Knowledge of relevant national guidance and other relevant initiatives.
8. The ability to work in multi-disciplinary teams across health, social and voluntary sectors.
9. Excellent communication skills.
10. Motivation, adaptability, and innovation skills.
11. Access to a car and hold a valid UK license.
12. Confidence using IT systems and a working knowledge of Microsoft Office packages (e.g., Outlook, Word, Excel).
