CHANGING FUTURES TEAM LEADER, NOTTINGHAM, 12 MONTH FIXED TERM, 37HPW, FRWK13 - £29,396 £32,716, FTE Changing Futures exists to improve the lives of people experiencing Severe and Multiple Disadvantage (SMD) in Nottingham City. Severe and Multiple Disadvantage (SMD) is generally considered to be an experience of three or more of the following sources of disadvantage simultaneously: mental health issues, homelessness, problematic substance use, domestic abuse and contact with the Criminal Justice System. The frontline delivery team is made up of Navigators and Specialist Navigators (working with minority groups and survivors of domestic abuse), a Housing Led Team with caseworkers, and embedded practitioners. There is also a Lived Experience Team, dedicated Peer Mentors and an Experts-by-Experience Board. Underpinning the frontline work, there is a Learning and Evaluation Team and city-wide learning resources via the Nottingham-based Practice Development Unit; all of which is overseen by a dedicated System Change Commissioner. The Navigator team hold a caseload and directly support people to access services. The successful candidate will work with beneficiaries to create individual support plans and liaise with other supports and agencies. You will be supporting a group of people who will have struggled to access the services they need; therefore the team and its members need to be creative, flexible, patient, persistent and assertive in their approach. Changing Futures is looking for a Team Leader to support operational delivery and line manage a team of navigators Duties and Responsibilities: To assist the Project Manager in managing the service so it achieves and maintains satisfactory performance and meets contractual requirements To line manage a team of frontline navigators. To ensure that all Navigators support plans, risk assessments and NDTs are completed in line with established timescales and meet the required quality standards To triage referrals and manage referral meetings/process Other managerial tasks as appropriate.
