Job summary An exciting opportunity has arisen for a highly motivated Advanced Nurse/Clinical Practitioner to join North Cumbria Primary Care Limited. Based at the Workington GP Practice you will work independently, be expected to work as part of our new total triage team and seeing patients in their own homes. This role requires working 12-hour shifts on a flexible rota, including working a share of weekends on a rotational basis with other Advanced Nurse/Clinical Practitioners. The shifts schedule includes Weekdays - 07.45am 20.15 pm Weekends 10am 4pm. You will implement a range of care interventions including prescribing for patients presenting with un-differential and undiagnosed problems. MSc in Advanced Clinical Practice and independent prescribing qualification are essential for this role. The Advanced Nurse/Clinical Practitioner will be fully supported by our GP team but will always aim to work independently and where appropriate, in collaboration with other health professionals Main duties of the job Be able to demonstrate safe and competent clinical decision making, critical thinking and expert care Provide advanced levels of clinical practice, knowledge and skills Actively engage patients and carers in the management of their health, promoting health and well being Undertake history and physical examination to accurately assess the patients condition Request, interpret and act upon relevant diagnostic test and procedures Request, process and interpret pathology and other tests results as required. Support the team in dealing with clinical emergencies Prescribe medication in accordance with current legislation Maintain comprehensive, contemporaneous, and accurate nursing/medical records and documentation, utilising information technology where available to facilitate caseload management Manage wide-ranging and sometimes complex presentations Functions at a high level in relation to communication skills through patient consultation and nursing assessment of physical, psychological and social needs Have a flexible approach to practice in response to changing service needs and priorities.
