Care Manager - Home Care Packages, Community Support Options
Join Monash Health Aged Care Services as part of our Community Support Options team, delivering essential care and support to carers and recipients across the Casey, Cardinia, Greater Dandenong, Kingston, and Bayside municipalities. We are offering an exciting opportunity for a qualified and motivated Case Manager to join our Home Care Package (HCP) program, which is aligned with the latest aged care reforms.
Key Responsibilities:
1. Deliver a re-enablement and wellness-focused model of care, managing a caseload of HCP clients.
2. Conduct comprehensive client assessments and develop individualised care plans to support consumers in their homes.
3. Provide holistic care within a case management framework.
4. Collaborate with rostering staff, agencies, contractors, and key stakeholders to ensure quality service delivery.
5. Contribute to the ongoing development of the Home Care Program, including procedure planning and reviewing client services.
6. Meet regularly with the Care Manager and Team Leader to report on KPIs and budget performance.
7. Coordinate with multiple service providers to ensure care aligns with client care plans.
8. Monitor and review client needs and well-being and recommend appropriate services.
9. Manage new customer inquiries, referrals, and client details on the My Aged Care Services Portal.
10. Solve client-related issues by providing practical, client-centred solutions.
11. Stay updated on aged care reforms, legislation, and quality standards, and apply this knowledge to your daily work.
12. Prepare and manage consumer budgets.
13. Maintain a proactive and client-focused approach in all interactions.
About You:
1. Relevant tertiary qualifications in health or social sciences.
2. Proven experience in case management, particularly within aged care.
3. Strong understanding of aged care reforms and client-directed care models.
4. Excellent computer, communication, and organisational skills.
5. Ability to work autonomously and as part of a team.
6. Passionate about delivering high-quality, client-centred care.
