We are looking for anenthusiastic team worker who values communication with colleagues andpatients. We embody the concept of a full multidisciplinary team including GPs,Clinical Pharmacist, Advanced Paramedic Practitioners, Nurse Practitioner(who is dedicated to Care Home management), Physician Associate, Social Prescriber and afull Nursing team including Mental Health Nurses, HCAs etc. Weare passionate in our intention to help build the future of General Practiceand currently have both Physician Associate and Medical Studentplacements. We are keen to start GP training and are open to new ideas.
Main duties of the job
The main responsibilities of the role include:
1. Undertaking a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion;
2. Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation;
3. Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems;
4. Screening patients for disease risk factors and early signs of illness;
5. Developing care plans for health inconsultation with patients and in line with current practice disease management protocols;
6. Providing health education;
7. Recording clear consultation notes to agreed standards;
8. Collecting data for audit purposes and complying with QOF requirements;
9. Delivering enhanced services;
10. Compiling and issuing computer-generated acute and repeat prescriptions;
11. All general duties and responsibilities associated with a GP working within primary care.

Clinical responsibilities:
12. In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
13. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
14. Assessing the health care needs of patients with undifferentiated and undiagnosed problems
15. Screening patients for disease risk factors and early signs of illness
16. Developing care plans for health in consultation with patients and in line with current practice disease management protocols
17. Providing counselling and health education
18. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
19. Recording clear and contemporaneous consultation notes to agreed standards
20. Collecting data for audit purposes
21. Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
22. Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
23. In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

A medical practitioner whose name is included in the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003 Not subject to suspension under section 41A of the Medical Act 1983 Qualified General Practitioner (completed certificate of Completion of Training CCT) Currently on a PCT performers list and not suspended from that list or from the medical register DBS clearance
