They are part of a unique Partnership between the two Nursery Schools, Rainbows (Additionally Resourced Provision for children with ASD) and the Family Wellbeing Centre. Curzon Site: A Maintained Nursery School NEG2 provision Family Wellbeing Centre Fawood Site: A Maintained Nursery School An Additionally Resourced Provision (ARP) NEG2 provision Family Wellbeing Centre We are recruiting for a Triage Officer to work within the Family Wellbeing Centre team. Working under the direction of the Centre Manager, the successful candidate will: Deliver effective triage by establishing the nature of enquiries at first point of contact, by completing the FWC getting to know you assessment tool with families and supporting them to access the appropriate support and advice within the Centre or to other local services, at their first contact. To complete a rapid Early Help assessment with those families whose screening tool indicates more complex needs. To develop detailed action plans in response to the whole family needs, ensuring that family members have access to the right service at the right time and to refer on to the most appropriate service or practitioner to meet the families’ needs To maintain accurate records, including entering information on the Centre database and recording case notes on Mosaic to agreed standards, when required to do so. Sound working knowledge of Safeguarding issues, Policies, Procedures and Managing Risk Experience of direct work with families who are experiencing multiple disadvantage.
