JOB DESCRIPTION PostLead Practice Nurse Salary GradeTBC Reporting toManaging Director Accountable toPartners PURPOSE OF THE ROLE To lead and manage the nursing team within the practice, ensuring the delivery of high-quality, evidence-based patient care in line with clinical guidelines and practice objectives. The role involves supporting professional development, ensuring compliance with regulatory standards, and contributing to the strategic goals of the practice. MAIN DUTIES AND RESPONSIBILITIES Leadership and Management Act as a clinical leader and role model, demonstrating best practices and fostering a culture of professionalism, compassion, and teamwork within the nursing team. Lead the nursing team in the planning, implementation, and evaluation of care, ensuring alignment with the practice's overall goals and priorities. Manage the recruitment, onboarding, and ongoing professional development of nursing staff, conducting regular appraisals and identifying training needs. Develop and maintain policies, procedures, and clinical protocols to ensure safe, effective, and consistent care delivery in compliance with national and local guidelines. Oversee workforce planning within the nursing team to ensure adequate coverage during periods of absence and seasonal demands. Monitor and evaluate the performance of the nursing team, providing constructive feedback, coaching, and mentoring to ensure continuous improvement. Clinical Care Deliver a wide range of nursing services, including chronic disease management (e.g., diabetes, COPD, asthma), wound care, cervical cytology, immunizations, and travel health advice. Perform comprehensive health assessments and develop individualized care plans in collaboration with patients, families, and other healthcare professionals. Lead on specialist clinical areas, such as diabetes, respiratory health, or women's health, providing expert advice and acting as a point of reference for the team. Administer treatments, medications, and vaccinations in line with practice policies and national guidelines, ensuring accurate documentation of patient records. Support patients in self-management of their health conditions through education, lifestyle advice, and tailored interventions. Provide minor illness and injury triage and treatment within scope of practice, escalating cases to GPs or other clinicians as appropriate. Service Development Contribute to the strategic planning and development of nursing services, ensuring alignment with the practices goals and the needs of the patient population. Monitor patient outcomes through audits and clinical data analysis, using findings to identify areas for improvement and implement service changes. Support the introduction and integration of new technologies, tools, and systems to improve efficiency and patient care, including telehealth and digital health platforms. Lead quality improvement initiatives, working collaboratively with other team members to enhance the overall patient experience and practice performance. Stay informed of national and local health policies, advising the practice on their impact and ensuring nursing services remain compliant and relevant. Education and Professional Development Facilitate learning and development opportunities for the nursing team, including organizing training sessions, workshops, and shadowing experiences. Mentor and support newly qualified nurses, practice nurse trainees, and healthcare assistants, fostering their growth within the team. Maintain personal professional development and revalidation requirements, participating in relevant training and keeping abreast of clinical and technological advancements. Promote a learning culture by encouraging reflective practice and participation in clinical supervision. Health and Safety Act as the practice lead for infection prevention and control, ensuring adherence to national guidelines and carrying out regular audits. Develop and implement health and safety policies, including risk assessments and emergency procedures, to protect staff and patients. Ensure the safe handling, storage, and disposal of medical supplies, medications, and hazardous materials in line with legislation. Monitor and report any incidents, significant events, or near-misses, taking action to address root causes and prevent recurrence. Safeguarding and Equality Act as the safeguarding lead or deputy, ensuring concerns are escalated and managed appropriately, and staff are trained in safeguarding protocols. Promote equality, diversity, and inclusion within the practice, ensuring fair and equitable care for all patients, irrespective of background or circumstances. Administration Ensure accurate and timely documentation of all patient interactions, clinical assessments, and treatments in the electronic health record system. Maintain and review clinical data to support audits, quality improvement projects, and regulatory compliance requirements. Coordinate and manage recall systems for chronic disease reviews, immunizations, and health screenings to ensure patients receive timely care. Oversee the maintenance and updating of patient group directions (PGDs) and ensure staff are trained and authorized to use them. Assist in the preparation and submission of reports, including those for the Quality and Outcomes Framework (QOF), CQC inspections, and other regulatory bodies. Manage the ordering, stock control, and maintenance of medical supplies and equipment, ensuring cost efficiency and adherence to budgets. Collaborate with the practice manager to ensure the nursing teams rota and workload distribution align with practice needs. Respond promptly to patient and staff queries, addressing concerns and complaints in line with the practices policies. Mobility Employees may be required to work at any of Village Health Groups sites.
