Norfolk and Waveney ICB are excited to offer this opportunity for a highly motivated, experienced, and enthusiastic Registered Nurse with a specific interest in compassionate end of life care. You will assess and evaluate patient conditions in accordance with the National framework for NHS Continuing healthcare, fast track pathway.
You will arrange personalised care, which will require close monitoring, and work closely with individuals and their families during a difficult time. You will be part of a team that strives to deliver a high standard of care.
You will undertake face-to-face visits to complete regular reviews and DSTs where appropriate.
Main duties of the job
* Join the Continuing Healthcare Fast track team, managing and prioritising your own workload against the needs of patients and service requirements.
* Receive, triage, and accept referrals from appropriate clinicians for patients with a rapidly deteriorating condition who may be entering the terminal phase.
* Manage an individual caseload, supporting patients and their families with high-quality, safe, and compassionate care.
* Act as a specialist resource for colleagues and other multi-disciplinary team members, advising on Fast Track completion and appropriateness.
* Communicate effectively to share sensitive condition-related information with patients, providers, and carers.

* Good understanding of the health and social care environment.
* Skills for nurturing key relationships and maintaining networks.
* Problem-solving skills and ability to respond to sudden unexpected demands.
* Independent thinker with demonstrated good judgement and analytical skills.
* Experience in a similar role in the public sector.
* Experience in communications and stakeholder management.
