2. Monitor the quality of care they're receiving.
3. Be professionally accountable for the delivery of care.
4. Utilise technology to enable you to update records as you go.
Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill, or have physical disabilities. The service aims to provide an excellent standard of care that supports patients to remain in their home environment. In Northamptonshire, you will work as part of a multi-disciplinary team to support the service and focus on delivering quality outcomes and improving the patient experience. You'll play a vital role in keeping hospital admissions and re-admissions to a minimum and ensuring that patients can return to their own homes as soon as possible.

You will need to be adaptable and resourceful. You'll need to be well organised, confident, and able to cope in diverse situations to deliver patient care. Leadership and management skills are also important, as with all nursing roles, communication skills, problem solving, and good judgement are crucial. You will be part of a caring team and be able to work flexibly with minimum supervision, with the ability to prioritise work based on patient need.
Join us and become part of Team NHFT. NHFT is an integrated primary care and mental health Trust, providing physical, mental health, and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work, or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
