Exciting opportunity for an experienced Practice Nurse to join our friendly Plymouth PCN.
You will join a large team of Practice Nurses and Healthcare Assistants and will undertake all aspects of the practice nurse role including cervical screening, baby immunisations and chronic disease management.
We are an innovative 5 surgery PCN with a practice population of 34,000.
We are a training Practice where we encourage training and education for our own staff and we support student nurses, paramedics, physiotherapists and GP trainees.
Main duties of the job
The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policies and procedures.
Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes.
Providing specialist care for registered patients in the following key areas: IPC, Asthma, Hypertension, Diabetes, COPD, Cervical Smears, Contraception, Baby immunisations.
Providing routine treatment in general clinics, including Wound Management, Blood tests, and ECGs.
Person Specification
Qualifications
* UK Registered nurse with NMC;
* Minimum 2 years post registration;
* Special interest in diabetes at L6 or L7;
* Experience working in General Practice setting;
* Cervical Cytology;
* Child and Adult immunisations;
* Care of Long Term Conditions (LTCs);
* Experienced in Wound care;
* Demonstrate an understanding of audit;

* Competent in the use of Office and Outlook;
* Good interpersonal skills, both verbal and written;
* Time Management and timekeeping;
* Ability to work as a team member and autonomously;
