The Community Rehabilitation Service supports individuals in their recovery from mental ill-health. The role of the Rehabilitation Therapy Assistant is to work across the rehabilitation pathway, supporting in the occupational assessment of the health, social and wellbeing needs of the individuals using the service, as well as the delivery of appropriate interventions accordingly. The successful candidate will be able to work both autonomously as well as part of a multidisciplinary team (MDT), attending reviews, MDT meetings and other meetings whether clinical or not.
The Rehabilitation Therapy Assistant role will work under the supervision of the Occupational Therapist/Clinical Lead, and is paramount in the safe, consistent and effective delivery of the service.
Strong communication, organisational and clinical skills are essential, as well as a solid value base and the ability to role model professional practice and behaviour.
Main duties of the job
1. Provide occupational therapy assessment based on rehabilitation need.
2. Plan and ensure provision of appropriate treatment to promote physical wellbeing and support mental health recovery.
3. Liaise with colleagues, external organisations and stakeholders in order to co-ordinate appropriate intervention, care and support.
4. Work within scope of own training, experience, practice and applicable legal frameworks.
5. Demonstrate and embed Navigo Code of Conduct through attitude, behaviour, leadership and practice.
6. Take responsibility for time management whilst balancing sometimes competing priorities.
7. Deliver group and 1:1 interventions.
8. Work autonomously and as part of a multi-disciplinary team.
9. Attend clinical and non-clinical meetings.
10. Ensure high standards of administration and record keeping.
11. Work out in the community as well as inpatient units.

Job responsibilities
1. The postholder must have the individuals' needs at the fore at all times and use the agreed set of values and skills to underpin their day to day work.
2. Supervise and support junior staff (health care assistants) in their roles, providing advice and acting as a role model and mentor.
3. Work without direct supervision, but with someone available to refer to as and when required, either on or off site on a regular basis, and to take charge of an area in the absence of qualified staff, making judgements around individuals' conditions at that time and the requirements of the area.
4. Ensure that individuals engage effectively with the agreed therapy plan and across appropriate services provided on a regular and consistent basis.
5. Plan and supervise individual or group therapy sessions, in line with the agreed therapy plan or programme of care.
6. Provide regular and practical support to individuals and their carers/significant others, in developing and managing dignity and independence.
7. Provide support and assistance as necessary with daily living activities.
8. Positively promote independent living of individuals within the community.
9. Work flexibly with Key Workers, Care Coordinators, other relevant community support, individuals, and significant others to implement a plan of care.
10. Help individuals gain access to resources, including benefits and welfare rights.
11. Handle individuals' cash and valuables on a regular basis.
12. Help identify early signs of relapse by monitoring the service users' progress, level of functioning and mental state, and alert the appropriate staff involved in the individuals' care.
13. Assist the Care Coordinator/Key Worker and individual in developing crisis/contingency plans and advanced statements, as per local policy guidelines.
14. Maintain adequate records, as required by existing procedures, entering appropriate details onto the individuals' case notes as necessary.
15. Communicate routine information in an arena where there will be barriers to understanding, and to be able to use tact and persuasion in a non-aggressive manner.
16. Provide information on health promotion.
17. Ensure the individual understands and has a clear pathway of care across sector/agency boundaries with key contact points/named individual.
18. Attend and participate in agreed training, development, and supervision sessions as an individual and as part of the team.
19. Undertake such other duties as may be determined from time to time within the general scope of the post.
20. Adhere to health and safety legislation.
21. Support Occupational Therapy and Physiotherapist staff to facilitate specific groups based on individuals' interests and assessed needs.
22. Support Occupational Therapy staff in completing home assessments for discharge planning from inpatient settings.
23. Work with Occupational Therapy, Health and Wellbeing coach and Physiotherapy staff to follow a therapy plan/wellbeing plan, including but not inclusive of: Moving and handling, prompting independence and positive risk taking following assessment, healthy lifestyle advice and intervention.

Experience
* Recent and/or current work experience within a Mental Health or Social Care environment.
* An interest in and commitment to ongoing personal/professional development.
* A keen interest in mental health as evidenced by reading on the subject.
* Mental Health First Aid training.
* An awareness of Health and Safety in the workplace.
* Working knowledge of the benefits and housing systems.
* Understanding of occupational and physiotherapy interventions.
* Understanding of therapy plans.
Skills & Attributes

Qualifications
