An exciting opportunity has arisen for a proactive, dynamic, and inspiring Band 6 Physiotherapist to join the Mendip Community Rehabilitation Service. The role is based in the Mendip area of AONB covering Glastonbury, Street, Wells, and the surrounding villages, home to the famous Glastonbury Festival, Clarks shoes, and the smallest city in England. We are a Community Therapy team, consisting of Physiotherapists, Occupational Therapists, Rehab Assistant Practitioners, Rehab Assistants, and Pharmacy Technicians in Somerset. We have undergone a reset and are ready to go. Our aim is to help the local population maximize their abilities to improve their quality of life. We are changing as we move to an innovative neighbourhood model as part of an ICS. We have an emphasis on prevention, early intervention at the initial point of need, and long-term self-management to enable patients to live better lives in their home environment. We see people in a host of settings, led by their goals but to include: their own homes and local community, our community hospitals, and our local care homes.
Main duties of the job
The main aim of the Community Rehabilitation team is to enable patients to stay at home, recover, and re-engage with the important aspects of their life and prevent admissions to the acute hospitals. To provide high-quality patient-centred care and specialist rehabilitation to adults referred to the Community Rehabilitation Service.
To provide specialist advice regarding rehabilitation management of patients to families, carers, and other members of the MDT. Working as a multidisciplinary team supporting patients to achieve their goals. Supervising more junior members of the team.
Supporting with the running of outpatient clinics including neuro and falls and frailty.

Maintains good communication, liaison, and working arrangements with other Trust teams, Trusts, and external agencies.
To use communication tools (verbal and non-verbal) to gain valid informed consent with patients where there will often be barriers to effective communication e.g. loss of hearing, altered perception, expressive and receptive dysphasia, pain, fear. To have the capacity to work within a legal framework with patients who lack capacity to consent to treatment e.g. those with cognitive or speech difficulties.

Essential
* Diploma/degree/Masters in Physiotherapy
* HCPC Registration
* Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses
Desirable
* Membership of the Chartered Society of Physiotherapy
* Membership of relevant CSP special interest group
* Postgraduate accredited course in Care / rehabilitation of the Elderly and/or other relevant postgraduate courses.
Experience
Essential
* Proven postgraduate experience with extensive clinical experience relevant to the post.
* Experience of a wide range of clinical rehabilitation areas of practice to include elderly patients with complex multi-pathology, neurological, orthopaedics, etc.
* Experience of working as part of a multidisciplinary team
* Experience in a Community setting
* Experience of lone working
* Knowledge of clinical governance

* Experience of service development and review
