Job summary Pathway Coordinators are an integral part of the Outpatient team, looking after patients from point of referral to discharge, ensuring that the process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the patient pathway. The role would suit someone who is good at multitasking, and problem-solving with an empathetic nature, and who has a passion for helping patients. The Pathway team is currently made up of 7 staff members based in the Outpatients department at Dorking Community Hospital, as we are seeing the number of patients grow, we are looking to expand our team. Main duties of the job Pathway Coordinators are responsible for ensuring that the patient journey is both efficient and effective. This involves chasing reports, results and other documentation from GPs, hospitals, and third-party providers to ensure these are in place before appointments. When processing reports, the Pathway team will highlight any urgent findings to our clinicians to expedite patient care where required, the ideal candidate would therefore be someone who can remain calm and maintain a high level of attention to detail. The successful applicant will be able to help manage the workload efficiently and effectively on a daily basis to facilitate the patients care being as smooth and efficient as possible. We will provide the training necessary to use our in-house IT systems, however a good working knowledge of Microsoft Office would be beneficial. DUTIES AND RESPONSIBILITIES OF THE POST Prepare clinics for each clinician across all specialities ahead of their clinics, this will include details about their expected appointments, routine results that have been received, pathology results for filing and patient queries. For new patient appointments, thoroughly review the referral information provided by the GP, picking out investigations or tests that have already been completed and adding them to the patients electronic record. For follow up patient appointments, review the previous clinic letter and referrals out recommended and ensure these have been conducted and the results are available for review. Process post-clinic folder documentation by distributing and actioning the contents as required. Shared responsibility for managing Pathway shared email box and the shared tasks on our patient record system actioning them as appropriate within a timely fashion. Communicating and sending the results of diagnostic tests we have conducted in house to the patients GP within the specified timeframes. For results we have received from external providers for our patients, review the results, add to the patients record, and take the necessary follow up actions; removing patients from waitlists, booking follow up appointments for example. There may be urgent findings on reports that need actioning immediately, following processes to highlight this to the clinician and follow their guidance on the outcome required to progress the patients care. Chase reports, results and other documentation with GPs, hospitals and third-party providers, working proactively and independently to achieve required outcomes. Manage the incoming and outgoing image requests. Track patient pathways from referral to discharge, highlighting and resolving queries as they are encountered. Maintain excellent working relationships with colleagues, consultants and external providers. Maintaining accurate patient records by updating wait lists, updating tasks, saving delivery / read receipts, adding results and notes. Manage multiple tasks, prioritising own workload and working with team to ensure work is completed within target timeframes.
