An exciting opportunity has arisen for an enthusiastic, dynamic, and committed Nurse Practitioner to join our Award-Winning Norfolk Forensic Community Learning Disability (NFC-LD) team. This is a fantastic career development opportunity within the Adult Forensic Learning Disability community service working alongside an established and experienced team of clinicians and practitioners to contribute to high-quality service provision, including consultation and advice, completion of specialist risk assessment, assessment and formulation, therapeutic interventions, and delivery of training, all within the overall framework of the service's policies and provision.
You will be actively involved in supporting plans for accelerated transformation in forensic learning disability services with a focus on improving continuity of care for patients. The expectation is that this approach will not only lead to reductions in length of hospitalization for patients within forensic learning disability units, but also minimize the need for in-patient admissions in the first place.
Main duties of the job
This post is based within the Norfolk Forensic Community Learning Disability (NFC-LD) Team; however, a key feature of the team is its close working relationship with the Broadland Clinic, a Medium Secure Forensic Learning Disability Inpatient Service, which sits within the Learning Disability and Forensic Strategic Business Unit of Hertfordshire Partnership University NHS Foundation Trust. The team will be actively involved in the discharge planning process.
The Integrated Community Forensic Learning Disability team has multi-disciplinary participation. It has a stepped service offer which includes:
1. Training packages on a range of Forensic Learning Disability topics
2. Consultancy and advice to community learning disability teams and in-patient units.
3. Delivery of a range of community-based therapeutic interventions.
4. Continuity for people who get admitted to in-patient settings.
5. Monitoring progress and participation pro-actively in discharge planning to less restrictive settings.
6. Active case management of service users identified as having high risk forensic needs.
7. Delivery of effective evidence-based practice to adults with learning disabilities requiring specialist forensic services.

Job responsibilities
1. Support service users with a learning disability in the community, who have a forensic history and active risk profile, or whose risks indicate a high likelihood of offending without specialist management.
2. Collaborate with in-patient services to monitor service users' progress and participate pro-actively in discharge planning to less restrictive settings.
3. Function as a lead health professional and manage a caseload of people with learning disabilities and associated mental health needs, taking responsibility for the formulation of detailed, individualised, and person-centred assessments in partnership with individuals and families/carers.
4. Practice evidence-based offence-specific nursing interventions and treatment for offenders with learning disabilities within their caseload.
5. Provide high-quality, comprehensive specialist risk assessments that rely on Structured professional judgment (HCR-20v and ARMIDILO S) which includes assessment and formulation and sets out appropriate risk management options and recommendations.
6. Offer training to upskill local providers and community teams.
7. Increase the capacity and capability of community providers, families, and others to work effectively with people with a forensic history or forensic-like behaviour.
8. Identify the need for further assessment and complete this in partnership with the person and their network.
9. Develop links and collaborate with other mainstream services to support the various needs of individuals.
10. Maintain and promote practice within the Mental Health Act (1983), amended 2007, Mental Capacity Act, 2005, and Hertfordshire Safeguarding Adults frameworks.
11. Work with the clinical team and Team Managers to ensure high standards of clinical practice and seek continuous improvement.
12. Participate in the induction, training, and education of students and other staff as appropriate.
Person Specification
QUALIFICATIONS/EDUCATION/TRAINING
1. ENB 997/998 (Mentoring & Preceptorship or equivalent)
2. Postgraduate Qualification
PREVIOUS EXPERIENCE
1. Relevant experience of working with adults with a learning disability.
2. Experience working with service users with forensic needs.
3. Experience working within learning disability Assessment and Treatment settings, community-based learning disability teams, or similar.
4. Multi-disciplinary and inter-agency working.
5. Day-to-day management of staff at a supervisory level.
6. Experience working with service users with challenging behaviour, epilepsy, and mental ill health.

Salary: £37,338 to £44,962 a year, pro rata.
