Job summary
Brownlow Health is an at-scale Inner City Primary Careorganisation caring for around 80,000 patients across 4 sites. We are lookingto recruit an additional, experienced Band 8A pharmacist to join our pharmacyteam. Currently, this consists of several pharmacists, with two senior leads,technicians and prescribing clerks. Candidates must be independent prescriberswith relevant experience of Primary Care, and electronic prescribing.
The successful candidate must work effectively as part of largemultidisciplinary team, and provide patient consultations via multiplemodalities. This will include acute medication requests, prescribing queries,and medication reviews, including patients who are polypharmacy. The pharmacistwill be expected to cover an on-call pharmacy day, supporting the team of GPswith on the day EPS workload and queries.
The role advertised will further provide support the innovativeFrailty team that Brownlow have established. This will include home visits,attendance at relevant multidisciplinary team meetings, and providing supportfor the wider frailty team around polypharmacy prescribing.
Main duties of the job
To promote high quality, safe and cost-effective prescribing asinformed by evidence-based practice and national and local guidance etc. To ensure prescribing conforms to relevantlegislation.
To work collaboratively with the Brownlow Healths PrescribingTeam, the wider clinical team including GPs and nurses and support theBrownlow Health Frailty team.
To complete structured medication reviews for polypharmacypatients.
To work with the in-house Quality Team to support achievement ofthe Impact and Investment Targets (IIF), Direct Enhanced Services Contract(DES) and Quality and Outcomes Framework (QoF)
To respond in a timely manner to medicines safety advice ,from the Medicines and Healthcare products Related Agency (MHRA), anddisseminate to the wider team as needed
