Working under the umbrella of the Integrated Safeguarding Team in Cornwall, the Integrated Admiral Nurse Service works in partnership with Dementia UK and Cornwall Foundation NHS Trust. We are looking for a band 6 registered nurse with a real passion for dementia care with significant experience working with families affected by dementia with complex needs. The Admiral Nurse will work closely with families looking after their loved ones living with Dementia providing telephone liaison support, guidance. and working closely with teams across Cornwall proving advice, support and education. The post holder will support families in the Acute Hospital Setting, The Community Hospital Setting, and when people live in their own homes., including the transition into a care home. The Post holder will also have the opportunity to speak at conferences, provide educational training sessions, and speak at various public events. Will consider a secondment opportunity. Main duties of the job Advise on the care of people with dementia and their carers/families with complex care needs from diagnosis to post bereavement support, as part of the dementia pathway in Cornwall Provide holistic and specialist nursing assessment, using the Admiral Nurse Assessment Framework, for carers/families affected by dementia Responsibility for, and management of, a clinical caseload of families with complex needs as identified within triage assessment and supported by the Admiral Nurse Strategic Lead Provide a range of relationship-centred, bio-psychosocial interventions across the integrated care area, and delivered via tele-support. Work collaboratively with health, social and voluntary sector services to achieve coordinated care and promote health and wellbeing for people with dementia and their families Manage care transitions and support effective discharge for people with dementia and their carers/families to reduce frequency of attendance, admissions, and length of stay, but to also provide continuity of care during any transitions of care.
