Job responsibilities To become a member of a high functioning team Acute frailty delivering a quality comprehensive service to patients with Frailty. To work under direction of the specialist therapist to provide frailty assessment, clinical care and treatment of patients within the acute frailty pathway. Under direction of the specialist therapist you will work primarily within the emergency care floor and in-reach into acute wards, liaising appropriately with acute therapy teams in order to assess needs, providing same day services, as well as supporting the effective use of alternatives to acute care including SDEC, virtual wards and community rapid response services. Under direction of the specialist therapist for acute frailty, contribute to the delivery of frailty awareness activities across acute service.
