Senior Nurse – Enhanced Intermediate Care Service, Bracknell

We are looking for a Nurse ideally with recent relevant community clinical experience, who has a passion to work in the community, supporting people to remain in their own homes and live as independently as they can.
This post will suit a dynamic and flexible Individual who enjoys working as part of a strong Multidisciplinary Team to ensure quality standards are met for the people we support. The post-holder will work as part of the EICS Nursing team to help people achieve their SMART goals and will be responsible for an agreed specific caseload.

Bracknell Enhanced Intermediate Care Service is looking for an experiencedNurseto work as part of the multidisciplinary team.
The team consists of Physiotherapists, Nurses, Clinical Psychologist and Community Matrons that work as part of a larger Integrated Team supported by Occupational Therapists, Social workers, support workers, Administration Assistants and Complex Care coordinators.
The team provides intermediate care and physical rehabilitation for the adult population of Bracknell Forest area. The Multidisciplinary Team works across 7 days per week to support people in their usual place of residence to avoid unnecessary hospital admission or to support early discharge,Rapid Response as part of the7 dayAdmission Avoidance service.The Nursing roleswork 5 days aweekMonday-Friday inclusive. This could change in the future as the nursing team increases.
There is also an element of Discharge to Assess, right sizing to support social care provision, someEnd of Lifesupport andinput into the Falls prevention clinics and programmes.
Assessments are completed in patient homes. There is a largenursingcohort across East Berkshire which gives great learning and networking opportunities, and we hold personal development of our staff in high regard.
