These new roles will be part of our developing Transfer of Care Hub where you will work with colleagues from local authorities and the voluntary and community and social enterprise sector.
The Hub aims to ensure we provide people with the right care within the community so that they aren't in hospital any longer than is necessary.
Please note: This post is open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSW's are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.
If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.
Main duties of the job
As a Clinical Case Reviewer, you will play a crucial role in ensuring the quality and effectiveness of our mental health services. Your responsibilities will include:
* Influencing and leading on all aspects of discharge planning
* Providing expert advice to multidisciplinary teams
* Liaising between commissioners and providers
* Understanding and monitoring how the Mental Health Act in particular Section 117 aftercare arrangements are implemented
* Agreeing new plans of care
* Reviewing and where necessary leading on changes to existing packages of care
* Identifying areas for development to enhance patient care.
* Participating in audits and quality improvement initiatives.
* Providing training and support to clinical staff on case review processes and standards.
* Contributing to the development of clinical guidelines, policies and procedures
Minimum Requirements
* Registered healthcare professional (e.g., Nurse, Social Worker, Occupational Therapist)
* Extensive experience in mental health services
* Strong analytical and critical thinking skills.
* Excellent communication and interpersonal abilities.
* Commitment to continuous improvement and professional development.

1. Clinical Case Management
1. Identify and risk-stratify vulnerable high risk service users requiring new specialist placements, by working with clinical teams and commissioners.
2. Attend Care Programme Approach (CPA) ensuring a clear and appropriate discharge pathway can be worked towards, in a timely way, so as to maximise opportunities for gaining independence and moving back into the community.
3. Ensure commissioned providers are delivering against excellent quality standards of patient care and that outcomes for individuals placed in specialist settings are being achieved. Establish robust ongoing quality and outcome monitoring processes; taking appropriate action to improve any underperformance.
4. Monitor placement / care costings to ensure ongoing value for money, and that care and treatment delivered is reflective of service user need.
5. In emergency situations, where an urgent response is required, work in cooperation with local safeguarding, crisis and inpatient teams to ensure risk and crisis management plans are implemented and service users are kept safe.
6. Establish and manage a risk and issues tracking mechanism for monitoring all placements and care packages.
2. Information and Performance Monitoring
1. Work with the MH Information Analyst to devise and support improvements to current management information.
2. To record and report on baseline performance targets and engage in activity management plans and capacity reviews with commissioned providers.
3. Report key findings to commissioners to support service improvement and continued value-based commissioning.
3. Operational Responsibilities
1. Contribute to the strategic planning of Mental Health and Learning Disabilities projects.
2. Contribute to short, medium and long term business planning cycles.
3. Keep informed and abreast of new technological advances in telecare and telehealth.
Professional Registration
* Professional Registration with NMC or HCPC
Clinical Work Experience
* Knowledge & understanding of the MHA & in particular section 117 aftercare
* Experience working in collaboration across teams & organisations
* Experience & ability to review complex cases to determine eligible health needs
