We are excited to offer a full-time secondment opportunity of 12 months within the Watford planned care team.
We are looking for motivated, proactive, and enthusiastic individuals keen to develop and grow in community nursing.
This post offers experienced nurses the opportunity to develop or expand clinical skills, ranging from wound care, palliative care, and IV therapies while receiving support to develop leadership and managerial skills.
Scope to increase to full-time post.
Main duties of the job
1. Provide leadership and management to a team of community nurses.
2. Be responsible for the team’s assessment, planning, implementation, and evaluation of individualized nursing care to patients living in the community. The service is provided seven days per week from 8 am – 6 pm.
3. Maintain patient and other records.
4. Participate in the long-term management of patients within the locality.
5. Have continued responsibility for a defined caseload.
6. Demonstrate good communication and commitment to teamwork at all times.
7. Support with sickness and absence management in line with policy.

Qualifications
* Registered Nurse
* Mentor/Preceptor/998 or equivalent
* Specialist Nursing Practice (District Nursing) Degree or will undertake training
* Nurse Prescribing or will undertake training
* Knowledge of recent developments in NHS
* Knowledge of evidence-based practice
* Evidence of personal development
* Knowledge of NMC code of professional conduct
* Relevant courses in Long Term Conditions Management
* Course of Top to Toe examination/diagnosis Experience
Experience
* Management of a team
* Experience of teaching others
* Proven clinical skill at Senior Community Nursing level
* Experience of participating in investigation/management of complaints
