Main duties of the job
The recruitment team at the University Hospital Southampton NHS Foundation Trust (UHS) are delighted to be able to offer a fantastic opportunity for a customer focussed, friendly and enthusiastic individual, as part of our expanding service. The Myeloma service is looking for a motivated and organised individual to take on the new role of Patient Pathway Navigator. The role will involve acting as a centralised point of contact for patients, working alongside the clinical nurse specialist (CNS) team escalating patients' concerns as appropriate. You will also be pivotal in coordinating the care of patients through their hospital care journey, providing the patient with the relevant information and coordinate any test preparations that need to be sent or collected by the patient.

Job responsibilities
Your interpersonal skills will enable you to be able to quickly forge good working relationships within the hospital and make you someone our patients can trust to effectively co-ordinate their care.
You will be responsible for identifying any blockages or concerns in patient pathways, and recommending solutions and changes to current processes to improve service delivery and compliance.
Work closely with consultants, specialist nurses, clerical and other staff to deliver a co-ordinated, consistent management of patient cancer pathways. Escalate any clinical concerns to the appropriate clinician, CNS/Physician Associate.
This role is ideally suited for those candidates who are already working at a Band 3 level or above, and looking to develop themselves within the NHS, whilst actively working in and supporting multiple teams around them.
The successful candidate will have a good grounding in medical terminology and experience of working in a pathway coordination/navigation role or similar.

Qualifications, knowledge and experience
Essential
1. Training and experience in administrative roles to vocational level 3 or equivalent
2. Good knowledge across a range of administrative work procedures including non-routine elements to Diploma level 4 or equivalent
3. Good grounding in medical terminology.
4. Relevant experience in an NHS setting with a wide breadth of speciality knowledge.
5. Experience of working in a pathway coordination/navigation role or similar.
6. Proficient in the use of all hospital computerised patient systems.
7. Experience of working in a patient or customer facing role with ability to establish report.
Desirable
8. Service improvement training.
