Specialty Doctors in Geriatric Medicine and Acute Frailty
We are looking for Specialty Doctors in Geriatric Medicine to join our Ageing and Health Department. There are currently eleven part-time substantive Consultants, two fixed-term locum Consultants, and six Associate Specialists providing senior leadership to frailty services across the organisation. Services include the frailty hospital at home service and urgent community response team liaising with the Acute Frailty team including frailty same day emergency care, Frailty wards (120 beds), Orthogeriatrics, Stroke, inpatient Older Peoples Advice and Liaison service and Proactive care based at Milford Hospital.
We welcome applicants with at least four years' experience in general and geriatric medicine. We have developed local Tier 3 training to support career progression for Specialty Doctors and other allied healthcare professionals in Geriatric Medicine. We would envisage post-holders will rotate four monthly through different frailty services developing a portfolio of competences and progressing to become an Associate Specialist or a Consultant through the portfolio (formerly CESR) route applying for entry on the Specialist Register.

Main duties of the job
This is a substantive post at Specialty Doctor level as per the Specialty Doctor 2021 contract. It is likely to rotate every four months within Ageing and Health to ensure a wide range of experience. It currently has no weekend commitments but is likely to contribute to seven-day working in the future with day-time weekend duties (one in ten weekends).
Post-holders usually rotate four monthly through areas of our Frailty services including:
* Frailty Units (Hindhead, Elstead, Eashing and Milford)
* Stroke (Acute stroke unit / rehabilitation / frailty ward)
* Orthogeriatrics
* Acute Frailty team in emergency department and emergency assessment unit.
* Inpatient older peoples advice and liaison service (iOPAL)
* Milford proactive care hub

Job responsibilities
We are proud to have an enthusiastic and dynamic team of permanent Consultants, Associate Specialists and Specialty doctors who work closely together to deliver our overarching Frailty Strategy, which aims to improve the care of all patients living with frailty in Guildford and Waverley.
We are working to integrate acute and community frailty services and to provide seamless care for our patients. For those with emerging or stable frailty we aim to provide standardised proactive comprehensive geriatric assessment and for those with frailty crisis, an effective alternative to admission including urgent crisis response, same day emergency care and hospital at home / virtual wards.
These emerging services require additional workforce to deliver the service and to support developing effective frailty services going forward.

Qualifications
* Full GMC Registration with license to practice
* MBBS or equivalent
Knowledge and Experience
* Evidence of continuing professional development
* Has completed four years of training with two years in Geriatric Medicine or equivalent specialty
