Community Support Worker – Home Treatment Team

The Newbury Older Adults Community Mental Health Team is looking for an enthusiastic, caring and highly motivated Community Support Worker to join our friendly and innovative Home Treatment Team.
The Home Treatment Team in Newbury is a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community. The post holder will work alongside the Mental Health Practitioners within the team to support the implementation of intensive care and treatment in the home of people with functional mental health problems, as well as people with dementia.
About you: The successful candidate must display a passion and understanding for working with Older People; appropriate training and support will be provided as needed.
The normal rota hours are a combination of 9-5pm or 11-7pm, including weekend shifts which are 9-3pm once a month (appropriate supervision will be provided for evening and weekend cover).
The post holder will be well supported and guided by the Team Lead and Mental Health Practitioners within the Home Treatment Team (HTT). They will have their own allocated caseload and support the delivery of excellent care to our HTT caseload. They will provide high quality mental health interventions appropriate to the individual.
Main duties of the job
1. Observe and monitor mental state and levels of functioning and report observations to qualified mental health staff.
2. Talk to patients and their carers, being aware of their individual needs.
3. Provide social and emotional support to patients and carers within the context of trusting and therapeutic relationships.
4. Enable the patient to maintain and develop their social skills, encourage social contacts and promote social networks if appropriate.
5. Assist in the process of assessment and meeting care needs of clients and their carers in the community.
6. Participate in individual therapies in patients' homes and in the community, contributing to the jointly agreed care plan, working under the supervision of qualified staff.
7. Co-facilitate and support staff in group activities.
8. Provide support to MHPs in the provision of psychosocial assessment and intervention for behaviours that challenge in dementia.
9. Liaise with other agencies in both statutory and voluntary sectors, e.g. Alzheimer's Society, Age Concern, Social Services, etc.
10. Be responsible for organising own daily work schedule to meet the clinical needs of the service.
11. Maintain clinical records as requested by a qualified member of staff.
12. Participate in the process of clinical supervision and appraisal.
13. Attend team meetings as appropriate.
