Job summary Easington South mental health team caters to an area of practice in Co Durham region. Following a Kaizen event in February 2020, adult community mental health services in Durham/ Darlington underwent internal re-organization but due to Covid it was postponed until September 2021. The team function has changed from previously being Affective disorder team to now having become a generic CMHT. This model is in line with CMHT transformation and is better aligned to PCN. The team is based at Merrick House Easington, with a large community base shared with Access Service and Easington North Community Intervention team. The Team provides secondary mental health service input for all mental health conditions for working age adult population. We receive referrals mostly from GP surgeries via central Access team and other sources such as Crisis team/ Liaison Psychiatry/ wards. We cater to patients with significant mental health difficulties such as mood disorders/ schizophrenia/ bipolar disorder/ personality disorder and adult neurodevelopmental conditions such as ADHD/ Autism. The Team is able to obtain expert advice from Trust-wide specialist services which include an ADHD service; the Autism Spectrum Assessment Team and the Eating Disorder service. The Trust has access to the Regional Affective Disorders Service in Newcastle. Main duties of the job Clinical duties of post holder New patient assessments 1-2 per week Follow up clinics. Approximately 4-6 patients per week Periodic Home visits. 1-2 per month MDT meetings. Formulation meetings. Clinical leadership. Mental Health Act assessments. Rarely, 1-2 per month. Out of hours middle tier rota. Clinic load will be kept relatively free to accommodate travel/ assessments involved. The post holder will be expected to provide clinical support to the team in conjunction with the Team Manager, Team Psychologists and Advanced Practitioners. They will attend team meetings and the leadership team meeting with Team Manager and Advanced Nurse Practitioner. Post holder will be working alongside Consultant Psychiatrist Core trainee and Non-medical prescribers in the Team. There is no inpatient commitment, but close liaison with In-patient Consultant colleagues is expected regarding individual service users if admitted to an in-patient bed.
