2. Position of the role in Claire House The post holder reports to the Care Coordination Lead nurse, and is accountable to the Director of Clinical Services 3. Scope of Job To act as a resource for all members of the multi-disciplinary team providing supervision, guidance, care and support To act as a contact worker throughout the child and or young persons Claire House journey demonstrating core, advanced communication skills depending on the specific needs of the child and or young person and their family To assess, plan, implement and evaluate the clinical/emotional requirements of children and or young people with complex, palliative and end of life care needs To liaise with external multidisciplinary agencies to ensure the care provided through the Hospice is coordinated and delivered to meet the specific needs of the child, young person, and family 4. Qualification and Skills Level Professional background as a Registered Nurse Child, Adult or Learning Disabilities Knowledge and experience in paediatric palliative care Effective range of clinical skills and or knowledge Post registration experience in relevant pediatric or young adult environment PART 2: MAIN DUTIES AND RESPONSIBILITIES 1. Responsibility for Staff To act as a positive role model by displaying CH values, attitudes and behaviour. In clinical practice, role model by using evidence and or research based care and practice To participate in in-service training programmes and teaching sessions To participate in the orientation of new staff, students and volunteers. Acting as mentor and or sign off mentor as and when required To support the appropriate delegation of duties to other members of the care coordination team To work collaboratively with other teams in a professional and supportive manner 2. Responsibility for Patient Care Be a main point of contact through the child and or young persons Claire House journey, including pre and post planned and unplanned stays and day care sessions To support the management of a large caseload of children and or young people, maintaining regular contact and liaising as required with key professionals Refer to internal spoke services and support the monitoring of internal offers of care Have an understanding of the planned and unplanned offers of care Working in partnership with parents and or carers to identify the care needs of the child and or young person and plan care recognising best practice and evidence based guidelines To liaise with therapists, learning disability nurses and specialist groups in order to produce an individualised care plan incorporating an individuals care, moving & handling and behavioural assessment plans To support the assessment, planning, implementing and evaluating care based on current evidence-based and researched best practice. Amendments will be negotiated with the child/young person and their parents and or carers, documented and communicated to the other members of the team and external professionals To have strong communication skills, able to adapt to differing needs of children, young people, and families To ensure all children and young people receive the right care by the right service for the right amount of time To accurately evaluate and document the care of the child and or young person in accordance with NMCs guidance on documentation To share responsibility within the Hospice in order to maintain a safe but homely environment taking into account health and safety, including infection control and risk assessments To ensure patient safety by appropriate delegation of duties to suitably trained members of staff To attend regular meetings with the multi-disciplinary team to discuss individual children, respecting and inviting the sharing of skills and expertise To liaise with outside Agencies from health, education and social care involved in the care and support of children and or young people and their families To work alongside, supervise and induct new members of staff ensuring clinical tasks and work load are delegated appropriately Demonstrate a level of knowledge of Claire Houses core and extended clinical competencies To participate and attend Multi-Disciplinary Team meetings in the community setting To demonstrate a level of knowledge of safe administration of medicines ensuring medicine management policies and procedures are adhered to To participate in hospice special interest groups To participate in internal referral panel and caseload review meetings To adapt the delivery of care according to the changing emotional, physical and environmental demands To be aware and working to Safeguarding and Prevention of Vulnerable Adults policies and procedures To comply with moving & handling policies and procedures To be able to undertake the mental, physical and emotional demands of the role, whilst at the same time taking care to safeguard their own health and safety as well as fellow care team members, children, young people and their family 3. Responsibility for Clinical Governance and Quality To take responsibility for maintaining their own professional development. Meeting with lead nurse for regular appraisals, identifying and agreeing personal targets To participate in quality measures for the delivery of care to maintain high standards through best practice To be proactive in identifying ways of improving practice. Contributing positively to the development and audit of standards and, when appropriate, taking responsibility for identified project work To identify ways of continued quality improvement, using reflective practice through Clinical Supervision To participate in audits to enhance the delivery of care To actively complete clinical incident documentation in accordance to no blame and whistle-blowing policy To adhere and comply with all Hospice policies and procedures To undertake risk assessments within the hospice and appropriate training To undertake research as appropriate to area of work 4. Responsibility for Clinical Leadership and Practice Management To share previous knowledge and experience that promotes best practice To ensure the economical and appropriate use of all resources within the Hospice To maintain appropriate client records ensuring they are stored securely and confidentially To attend and participate in Hospice meetings To attend, actively participate and learn from significant event analysis To report and record all accidents and incidents in line with hospice procedure safe guarding the welfare and safety of all staff during working hours with regard to the Health and Safety at Work Act To be fully involved in the process of preventing and controlling infections To maintain confidentiality in all aspects of the role, especially information regarding children and or young people, their families and staff 5.
