We are looking to recruit a Band 5 Mental Health Practitioner (Mental Health Nurse, Social Worker or Occupational Therapist) to the Early Intervention Team.
We are a team that works with clients experiencing their first episode of psychosis. We are a multidisciplinary team that covers the whole of Sheffield.
Main duties of the job
Clinical:
1. With colleagues, carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree on the most appropriate course of action.
2. To assess, evaluate and manage clinical risk.
3. To formulate a plan of care based on identified need.
4. Deliver evidence-based interventions as appropriate.
5. Review the effectiveness of the care plan at regular intervals.
6. To work collaboratively with service users, carers, and partners.
7. To maintain computer-held care records in an accurate and timely fashion.
8. Undertake all work in a culturally sensitive manner.
9. Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.
10. Develop effective working relationships with Primary Care colleagues.
Supervision:
1. Regularly review caseload to promote efficacy and contribute to the overall management of Service.
2. Deliver supervision and facilitate PDR with more junior members of staff.
3. Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution.
Education and Training:
1. Act as a mentor to learners, keep up to date with relevant course and curriculum developments, and maintain 'sign off' status.
2. Participate in training relevant to the role.
