The team works closely with colleagues across Oxfordshire to provide support for patients in their own home environment. This can be during an acute illness, to prevent hospital admission, or to facilitate discharge from hospital. We are developing our frailty service which focuses on providing elderly frail patients with support and care planning to help them stay at home.
