Wootton Medical Centre is seeking to recruit a highly motivated Practice Nurse (CDM Nurse / Treatment Room Nurse) to join our friendly, innovative team who are committed to delivering high standard of patient care. We are a busy village surgery with over 7,800 patients, 5 GPs, Paramedics, Physiotherapist, 4 Nursing Staff and our Reception / Administration team.
We are looking for a Practice Nurse (NMC registered), you will have previous experience working in primary care and experience of chronic disease management.
We are a supportive team and work together to ensure our patients receive the best care and support possible. You will work as a member of our multidisciplinary team and will be fully supported by clinical and non-clinical staff.
The role is for 14 hours over two days.
Main duties of the job
The successful Practice Nurse will work in collaboration with the nursing team, managing patients with chronic disease, in particular asthma and COPD, in line with NICE guidelines and quality outcomes framework (QOF), infection prevention and control, health promotion and reviewing clinical policies and procedures.
Duties will also include cervical swabs and screening, travel health advice and vaccinations, child immunisations, audit and stock management and standard treatment room duties such as venepuncture, BP checks, urinalysis, dressings, suture / staple removal, ear syringing, spirometry, weight management, pill checks, injections and vaccinations.

Job responsibilities
To be responsible for the delivery of evidence-based nursing services, working as part of the surgery multidisciplinary team, delivering care within the scope of practice to the entitled patient population. Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as health prevention and promotion, chronic disease management, patient education, well women / man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
Duties & Responsibilities:
* Assist with the organisation and co-ordination of the provision of nursing services for the surgery;
* Provide assessment, screening treatment, services and health education advice;
* Develop, implement and evaluate individual treatment plans for acute, non-acute conditions;
* Support the clinical team in the development of individual treatment plans for chronic disease patients;
* Provide chronic disease clinics, delivering patient care as necessary, referring patients to GPs, secondary or specialist care as required;
* Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health;
* Deliver nursing care according to NSF, NICE guidelines and clinical-based evidence care;
* Prescribe medication in accordance with BNF and within own scope of practice;
* Review medication for effectiveness in accordance with patient need and national guidance;
* Provide information and advice on prescribed and/or over-the-counter medication on side-effects, common interactions and dosage;
* Support patients to ensure compliance with prescribed medications;
* Provide wound care (Doppler / leg ulcer etc.) to patients;
* Be competent in administration of childhood immunisation and travel vaccinations and implement and participate in vaccination and immunisation programmes for both adults and children;
* Request pathology services as necessary, processing and interpreting results as required;
* Undertake the collection of pathological specimens, including intravenous blood samples, swabs, smears etc.;
* Maintain and control injectable and oral drug stocks;
* Maintain accurate clinical records consistent with legislation, policies and procedures;
* Maintain chronic disease registers ensuring read codes are used effectively;
* Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care;
* Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment;
* Recognise and work within own competence and professional code of conduct as regulated by the NMC;
* Prioritise, organise and manage own workload in a manner that maintains and promotes quality, ensuring effective time-management strategies are embedded within the culture of the team;
* Oversee the duties undertaken by the HCA and assist with training and mentoring as and when required, and contribute to the implementation of an effective training programme for staff and students within the surgery;
* Support the clinical team with all safeguarding matters, in accordance with local and national policies;
* Continually review clinical practices, responding to national policies and initiatives where appropriate;
* Be aware of and contribute to the planning and implementation of CCG patient pathway initiatives.
