Wearside Medical Practice is a well-organised, forward-thinking, and friendly Practice with an approximate list size of 8300 patients. We are a busy Practice situated in the heart of Sunderland. We are looking forward to welcoming a Practice Nurse to expand and enhance our team, working full-time.

Main duties of the job
As well as general practice nursing duties (including, but not exclusive to, women's health and screening, vaccinations, injections, and health promotion), this post has a substantial focus on delivering effective individualized care to patients with long-term conditions within our new multi-condition review system.
You will have a high level of expertise in chronic disease management, particularly experience in managing asthma, COPD, CVD, diabetes, heart failure, and hypertension. You will be aware of and focused on meeting QOF targets while confidently delivering patient-centered reviews and supporting self-care and management.
Experience in filing and actioning test results would be advantageous but not essential.
You will help support the lead nurse in the training and development of junior members of the nursing team.

We currently have 1 GP Partner, 3 salaried GPs, 2 Advanced Nurse Practitioners supported by 2 Practice Nurses, 1 TNA, and 1 Health Care Assistant, along with a full complement of administrative staff.
We were rated overall Good by CQC in May 2022.
The Practice maintains a very high QOF achievement each year.
The Practice uses EMIS Web supported Accrux, eConsult, and GP TeamNet.
As part of the care home alignment scheme, the Practice holds weekly MDT meetings to improve care for patients with complex needs.
The Practice holds regular formal clinical and nursing educational/MDT meetings.
Job responsibilities
The post holder is a specialist nurse who has the knowledge, training, and experience equivalent to first-degree level and considerable evidence of post-registration development within the field of practice nursing.
The following responsibilities are seen as central to the above position:
Management of Health and Illness
* Assess, plan, and evaluate clinical nursing care to meet the needs of the individual and/or identified group.
* Assess, diagnose, and treat specific conditions in accordance with agreed professional protocols and guidelines.
* Manage and deliver programmes of care for chronic diseases.
* Receive direct referrals from healthcare professionals, other agencies, and patients.
* Prescribe medications within limits of personal competence as an independent or supplementary prescriber (if not a prescriber, consider this as an option for the future).
* Plan, manage, and deliver programmes to improve health and disease in individuals or groups.
* Engage with patients and professionals in evaluating nursing care and make recommendations for improvement.
Communication
* Set and maintain high standards of communication with patients, families, and carers.
* Set and maintain high standards of professional record keeping.
* Communicate clinical information clearly, succinctly, and effectively to other members of the clinical team.
* Establish effective communication within the nursing team and wider primary healthcare team.
* Support patients and carers who need assistance in exercising their rights.
Quality Improvement
* Contribute to the clinical governance activities within the team.
* Use current evidence of effectiveness to inform clinical practice.
