Job summary
Ward 31 at Darlington Memorial Hospital is a busy Trauma andOrthopaedic Unit caring for patients with trauma injuries and is also thedesignated area for patients admitted with head injuries.
The ward is a fast pace enviroment and can look after somevery complex patients.
The Discharge Co-ordinator role is to helpsupport the nursing team in ensuring that all patient discharges are maintainedto a high standard and also to assist with the complexities involved.
Main duties of the job
The main duties of this role will include:
Supporting nurisng staff with all discharges, including verycomplex ones.
Liaising with all parts of the MDT such as doctors, nurses,OT and physio.
Building good relationships and making referrals to socialservices and other support teams.
The role will involve a lot of autonomy and selforganisation.
Communicating with patients to support and facilititatethere discharge home.
Support Nursing staff with very complex anddifficult discussions around discharging concerns from hospital.

Job responsibilities
The role entails liaising with the Nursing staff on aregular basis and assisting with the discharge process of patients. The rolewill require you to take the lead on complex discharges and ensure that youhave liaised with the relevant teams to ensure that complex needs have beenreviewed and promote a safe hospital discharge.
You will be need to be able to workindependantly and with lots of autonomous practice. The role will also requireyou to be a good communicator and very organised with multiple patients andcomplex requirements. You will besupported overall by the nurse in charge on a daily basis.

Essential
1. Able to travel across Trust sites to meet the requirements of the post.
2. The role will be mainly 9:30am-5:30pm but flexiblity is required to adjust regularly in relation to service demands and also some weekend working where required.
3. On occasions you may be required to support the other Trauma and Orthopaedics and surgical areas within the trust on different sites.
Special Skills and Knowledge
Essential
4. Demonstrate the ability to liaise with other parts of the MDT including other care services such as social care.
5. Demonstrate the ability to make patient refferals.
6. Knowledge and undertanding of patient mobility requirements for rehab areas.
7. Good use of microsoft computer skills.
8. Teaching skills to help keep nursing staff up to date with the policies and procedures around discharge management.
Desirable
9. Previous Discharge Co-ordinator.
10. The ability to work automonously and make decisions around patient care needs.
Qualifications

11. GCSE Maths and English or equivalent.

12. NVQ Level 3 in Health or Social Care or equivalent experience and be willing to work towards.
