Job summary We are seeking a Care Coordinator to provide support within primary care to both patients and healthcare professionals. The Care Coordinator is a non-clinical staff member in a frontline patient-facing role. You will be required to work with 8 local care homes with both elderly and young people. You will form a connection with the care home staff and residents and perform a weekly ward round by visiting the home in person or via telephone/teams. Main duties of the job As a Care Coordinator, you will act as a point of contact for patients and carers, answering queries and signposting to relevant local services. You will also support practices with caseload management and identification, and will be involved in the coordination of MDT meetings. You will be responsible for various areas of data management including data analysis, auditing, coding, and ensuring patient records are correct and up to date. The role of the Care Coordinator goes beyond administration, requiring you to engage with patients and work within a network of local providers and organisations to ensure patients have access to the care they need.
