Job summary The postholder will be a member of the Community Learning Disability team, working closely with the multi- disciplinary team. Responsibilities include the maintaining of a clinical caseload and conducting assessments of highly complex problems which require the analysis, interpretation and comparison of a range of solutions for adults with learning disabilities and their families who are experiencing complex health difficulties, associated with their learning disability. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. This post is Fixed Term for 12 months to meet the needs of the service. Main duties of the job This role will include the maintaining of clinical standards within the governance framework.To work within their code of practice to take responsibility for their caseload, ensuring comprehensive assessments of their needs. This includes the formulation, planning, delivery and evaluation of care and will involve the regular review and updating of risk assessments, assessment documentation and care plan reviews.To work collaboratively across a wide range of multi-disciplinary services and agencies to provide effective delivery of care, supporting aprogramme of equal access to mainstream services with reasonable adjustments with collaborative interventions from learning disability services.
