Job summary The North West Anglia Foundation NHS Foundation Trust transition team is dedicated to developing a person-centred, developmentally appropriate service to support young people aged between 11 and 25 years with a long-term health condition(s). The post holder will work cross-site alongside the Transition Co-ordinator to advise & support young people & their families/carers as they navigate from paediatric to adult health services. They will help to ensure young people and their families/carers feel welcome, informed and have relevant, up to date and effective information and communication to facilitate their smooth transition to adult services. Main duties of the job To support the Transition coordinator in day to day tasks and service development To proactively engage with young people, their families and carers to ensure they have the relevant information, education and support to facilitate a smooth transition to adult services.
