Hospital Discharge Social Worker

We are looking to recruit a Qualified Social Worker for the Hospital Discharge team within the Social Care industry in Somerset Council. This vacancy will be for an initial 3-6 months, with potential for extension subject to internal circumstances and your performance.

Responsibilities include:

1. Working with Vulnerable Adults within a hospital setting
2. Report writing and making assessments
3. Developing care plans
4. Care management
5. Managing complex cases
6. Liaising with other professional bodies
7. Arranging safe and timely discharges for service users
8. Self-directed support / personal budgets

Experience preferred:
Post-qualifying experience of working within a Qualified Social Worker - Hospital Discharge team within the UK Social Care industry.

Minimum Requirements:
1. HCPC Registration
2. 1 year post-qualifying experience within the UK Social Care industry
3. Enhanced DBS/CRB (we can obtain this for you)
4. Eligibility to work in the UK
