We specialise in working with people with long term mental health conditions who live in block funded mental health supported accommodation. We aim to provide a range of support so that together, we can:
* Identify and build on positive things in the lives of clients
* Support client 'recovery' and achieve client goals
* Help clients to live as successfully and independently as possible within the community.
We provide a 'strengths based' model of care. We support people with mental and physical health through a range of interventions, including group activities, psychology, medicine and more. We are a small MDT including support workers as well as access to an employment specialist and welfare advisors. We work alongside clients, their carers, accommodation support workers, Southwark social workers, inpatient wards, GPs and more.
We are based just a short walk from Denmark Hill train station, Maudsley Hospital and King's College Hospital. There are lots of places to get lunch nearby. We are a small supportive team and we try to have lunches out together a few times a year.
Main duties of the job
* Coordinating the care of a caseload of clients with complex needs.
* Adopting and providing clinical support in a 'shared care model'
* Be actively involved in the planning and coordinating of the service users' journey into appropriate placements, minimising delays and maximising capacity.
* Gain awareness of the processes relevant to placement panels to support situations where an aspect of the discharge pathway is delayed or unknown.
* Have an understanding of the pathways between services including access to a range of service provision.
* Provide clinical advice and professional support to junior staff, students, service users, carers and accommodation workers.
* Have reasonable links and understanding of the social care aspects that would be relevant to support individuals with social care barriers to discharge.
* Contribute to the monitoring of our rehabilitation model within placements and of the overall patient journey promoting proactive use of collaborative tools and methodologies to facilitate efficient patient progression.
* Have direct communication with inpatient and other community teams and develop instrumental relationships that support the service.
* Continually maintain excellent written communication and data (KPIs) necessary to evidence a more responsive system of governance.
* To assist in QI including regular quality audits using Tendable. To encourage getting feedback using PEDICs.
