Salary: £34,342 - £37,196 per annum Contract: Permanent Location: Wildwood Drive, Worcester WR5 2QT Main duties of the job We are seeking caring and professional registered nurses to join our Integrated Community Service. The role will provide a responsive, flexible and high-quality community specialist palliative care service to patients and those important to them. Working autonomously in the community you will provide assessment of patients with complex palliative care needs and to develop management options, advising others involved with patients care and evaluate the plan as needs change. The role will involve working collaboratively and co-operatively with clinical colleagues across a number of teams which will include; Advice & Guidance, Rapid Response or in a cluster if required to contribute to the continual development of services and quality of care delivered. This post requires an Enhanced Disclosure and Barring Service check. You will be a registered nurse with 3 years post registration experience with experience of working within palliative care or experience of working within a community setting. A nursing degree or study at level 6 is required and you will have the ability to provide peer support and mentoring of others, demonstration of leadership qualities would be desirable but not essential. Due to the nature of the community role and working throughout Worcestershire, a full driving licence and access to a vehicle are essential.
