Contribute to admission avoidance through a multidisciplinary team approach and case management. To work across traditional professional and geographical boundaries and extending provision of support and advice to primary care, secondary care, local authorities and social care to ensure vulnerable patients are well supported with all their medicines needs. Provide highly specialist medicines management support and training for GPs, nurses and other members of the multi-disciplinary teams across the interface where appropriate. Provide highly specialist medicines advice and training to patients, carers, and care home staff on medicines related issues where appropriate. Empower and encourage self-care and independence by implementing robust self-administration support processes and undertake domiciliary visits where appropriate Working to improve healthcare outcomes for patients using a holistic approach to care, ensure the safe and effective use of medicines. Engaging with patients in primary and community health services to improve their experience of medicines, ensure their voices are heard and integral to any decisions made about their care Carry out physical observations, phlebotomy and undertake clinical examinations within areas of competence. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients with an undifferentiated diagnosis. To provide care for the patient from initial history, clinical assessment, diagnosis, treatment (including prescribing when required) and evaluation of care. Undertake telephone / video consultation / face to face consultations Undertake structured medication reviews and create pharmaceutical management plans. Liaise with PCN pharmacists and GPs to carry out the management plans once initiated. Monitor patient progress where appropriate. To utilise prescribing and de-prescribing skills as a non-medical prescriber in line with competency and agreed scope of practice as approved by the relevant Trust governance process and line management. Empower and encourage self-care and independence by implementing robust self-administration support processes and domiciliary visits where appropriate To proactively plan and initiate actions related to improving the use of medicines for better outcomes and reducing medicines waste across the primary/ secondary care interface and if needed, within the GP practice setting. Manage medicines related risks on an individual patient basis and in line with the local and national patient safety agenda. To develop outcome measures through data collection and effective audit processes with a view to extending the service to other practice areas and patient cohorts as deemed appropriate. Developing quantitative and qualitative performance measures, co-ordinate their use and be involved in disseminating and acting on the information gathered.
