Job overview
To provide comprehensive, efficient and effective administrative service within a Transfer of Care Hub to support the discharge system and data outputs as required by its multi-Disciplinary teams both internal and external.
Using your organisational, analytical, administration and people skills, you will contribute towards an effective and consistent level of service. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
The role provides a fantastic opportunity to develop within an ever expanding service working with differing health care professionals and senior team members to help shape service improvements.
Main duties of the job
1. Update, collate and share complex discharge data within the service unsupervised.
2. Organise and prioritize daily tasks unsupervised using my own initiative.
3. Input and maintain information on the service’s various systems, ensuring accuracy.
4. Create and maintain data logs using MS Excel and linked NHS systems.
5. Create new records and monitor patients added to or removed from discharge logs.
6. Receive data, reinstating previous records or adding new referrals ensuring accuracy.
7. Data entry including retrieval and maintenance of patient records.
8. Deal with a high volume of queries via secure email, liaising with both internal and external agencies.
9. Effective liaison with multi-department staff, patients, carers, and outside agencies.
10. Answer the telephone, clear answerphone, record messages and respond to queries.
11. Collect and input data on to the Emis Web system, investigating discrepancies.
12. Participate in service development activities and mandatory training as required.
13. Assist with training of new clerks within the service.
14. Liaise with colleagues at all levels.
15. Manage workload and prioritize tasks unsupervised.
16. Operate and maintain information systems ensuring high standard of information recorded.
17. Produce standardised reports and/or statistical information as requested.
18. Transfer relevant information to all health and social care providers.
19. Input accurate and timely information required for audit purposes.
20. Provide cover for and undertake duties of absent colleagues.
21. Support individuals’ equality, diversity, and rights.
22. Contribute towards service improvement initiatives.
23. Provide information and support to service users/carers as appropriate.
24. Contribute to the continued improvement and quality of the administrative support service.

Qualifications
