Job summary Aneurin Bevan University Health Board are seeking to appoint a Health Disability Activity Practitioner into an exciting and unique role that partners with Disability Sport Wales and Local Authority Sport Development teams (or equivalents). This post is fundamental in supporting more active lives for disabled people across Wales. Aneurin Bevan University Health Board are seeking to appoint a motivated and enthusiastic individual into this developing role. This health-board wide role forms part of a national service. Supported by a National Lead, the post will include the delivery of Health Disability Activity Pathway training to health and social care professionals across many disciplines; promoting the pathway at local and national events; and working with third-sector service partners to ensure disabled people are signposted to appropriate community-based opportunities to improve their health and well-being through increasing their levels of physical activity (including sport). Main duties of the job The post holder will lead the ongoing implementation, embedding, and delivery of the Health Disability Activity Pathway partnership across the Aneurin Bevan University Health Board area, working with health and social care professionals, pathway partners, and community and third sector organisations (including National Governing Bodies of Sport). Duties will include delivery of the pathway training programme to health and social care professionals, promotion of the pathway, and being the central point of contact for referrals for signposting received by the Health Disability Activity pathway team for the Health Board area. A key part of the signposting process is to work closely with sports development teams (or equivalent) in the local area this includes leading regular locality meetings to update on the pathway and the provision provided in the local area. This role targets achieving the outcomes of promoting the health and well-being of disabled people through increased physical activity levels, as detailed within the pathways national and local delivery plans. Whilst travel across the Health Board area will be necessary, base can be negotiated on appointment. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
