Lewes neighbourhood mental health team, formerly the ATS have an exciting opportunity for a social worker to join our developing service. As we move into community transformation and working more closely with our partner organisations we are looking for a dynamic, enthusiastic, flexible, innovative and experienced Social Worker to join us.
We are a friendly professional service based in the lovely seaside town of Newhaven. Newhaven, a gateway to Europe, is close to the South Downs National Park and Brighton City Centre. Brighton has a vibrant shopping centre that includes many independent shops, restaurants and bars, as well as the seafront, and good public transport links to London. There are many beautiful beaches stretching the length of Sussex with ample opportunities for water sports.
Main duties of the job
You will need to be flexible and adaptable as modern mental health services are constantly changing in response to best practice. The post will involve working with clients on CPA with complex needs and their families.
You will be required to have strong communication and people skills and demonstrate a desire and ability to meet community service demands in an ever changing landscape. You will be working closely with existing current team members in identifying best use of resources in a needs led service utilising patient focused evidence based interventions.
The successful candidate will join a very busy and evolving multidisciplinary team. They will be able to demonstrate; excellent clinical skills, being able to work under pressure, and with MDT colleagues managing patients who present with high levels of acuity and risk.
1. To be Lead Practitioner and coordinate care for caseload of the most complex/ high risk patients

Detailed job description and main responsibilities
* The post will be working with patients in a community setting as part of the multi disciplinary team providing assessment and treatment.
* There may be some follow up under social supervision of patients in the community.
* Carry out a range of social work responsibilities within frame work of NHS and Community Care Act.
* The post holder will be conversant with legal duties under MHA 1983 and MCA including Safeguarding of Adults at Risk and Children.
* Have Social Circumstances report writing skills.
* As part of multi disciplinary team attend ward rounds, CPA and undertake analysis related work.
* Understand multi agency information sharing in the interest of public protection and MARAC.
* Hold a case load of patients.
* Visit families for information gathering and to offer support and provide carers assessments where needed.
* Visit patients in out of area placements to attend CPAs, MHT, Case Conferences and for future discharge arrangements.
* Be conversant with S117 Aftercare arrangements.
* Be a member of a multi-disciplinary team.
