Job Responsibilities:
1. Clinical Responsibilities:
* Undertake assessment for patients within the community and those attending the surgery, using diagnostic skills and initiation of investigations where appropriate.
* Discuss patients needing referral to secondary care with the registered GP before making such referral.
* Ensure clinical practice is safe, effective, and remains within boundaries of competence.
* Advise patients on general health care, with referral to other members of the primary and secondary health care team as necessary.
* Work from the surgery and within the locality as a paramedic, caring for patients and families.
1. Maintain personal and professional clinical standards in accordance with the Health Professions Council Standards of Conduct.
2. Carry out planned consultations to record observations, create agreed care plans, and provide signposting to other services.
3. Communicate effectively at all levels within the team to ensure an effective service is delivered.
4. Maintain accurate, contemporaneous healthcare records appropriate to the consultation.
5. Deliver evidence-based care at the highest standards, ensuring high-quality patient care.
6. Enhance own performance through Continuous Professional Development and share knowledge and skills with colleagues.
7. Participate in the audit process, evaluation, and implementing plans and practice change to meet patient needs.
8. Collect pathological specimens and perform investigatory procedures requested by the GPs.
Administration and Professional Responsibilities:
1. Participate in the administrative responsibilities of the practice team.
2. Ensure accurate and legible notes of all consultations and treatments are recorded.
3. Keep the clinical computer system up to date with accurate details.
4. Ensure accurate completion of all necessary documentation associated with patient healthcare and registration with the practice.
5. Attend and participate in practice meetings as required.
Training and Personal Development:
1. Training requirements will be monitored by yearly appraisal.
2. Participate in the education and training of students and all members of the practice staff.
3. Maintain continued education by attending courses and study days as necessary.
4. Develop and maintain a Personal Learning Plan.
Liaison:
1. Work closely with reception and office staff to ensure smooth running of the practice.
2. Establish and maintain good liaison with other surgeries and agencies, including secondary care.
Meetings:
1. Attend and contribute to various practice meetings as requested.
